The Registered Nurse (RN) - Lead Health Educator job is a civilian position providing care to active duty military, military retirees, and their families. The position is based out of a Primary Care clinic in Balboa Park.

Registered Nurse (RN) - Lead Health Educator Job Overview:

  • Assess each patient and perform triage duties as assigned.
  • Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies.
  • Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes.
  • Provide treatment and discharge instructions upon patient release.
  • Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.
  • Provide a safe and clean environment for each patient.
  • Identify patient/significant other learning needs and implement appropriate measures to meet identified needs.

  • Assist in planning, provide clinical direction and provide instruction to LPNs/LVNs, nursing assistants and ancillary personnel.

  • Review Health Enrollment Assessment Review (HEAR) reports and PCM reports for high risk and frequent users of health care. Contact these patients to schedule education opportunities in a classroom format, in one on one appointments, or through other appropriate means.

  • Review HEAR Reports and PCM reports for delinquent preventive services, contact the patient, and schedule an appointment for the clinical preventive service.  Ensure that all enrollees receive their clinical preventive services at recommended age intervals. 

  • Use the HEAR group data to determine the health risk of the population and how to improve the overall health of the enrolled population.

  • Educate the enrolled population about the Healthwise handbook and how to use it for home care.

  • Use the PPIP Flow Sheet DD2766 medical records form for documenting clinical preventive services.

  • Provide health promotion and education opportunities including, but not limited to, tobacco awareness; tobacco cessation classes or appointments consistent with the MTF tobacco cessation program; and nutrition education and weight management.

  • Manage care for the patients of the primary care practice within the Population Health domain to promote effective education, self-management support, and timely health care delivery to help improve patient's health and wellness efforts.

  • Collect, analyze, interpret and evaluate data to assess trends and developments to target the population and deliver Health Promotion and Wellness (HPW) services directly to patients.

  • Be part of a multidisciplinary team that assesses health promotions risk, analyzes health data, develops content for educational programs, drafts health promotions policy guidance, and evaluates health promotions outcome measures aim to deliver improved patient care.  As requested, the HCWs will provide input and technical guidance on issues related to the Health Promotion and Wellness (HPW) program.

  • Coordinate and deliver reports to BUMED Population Health teams.  Collaborate and communicate between BUMED Populations Health and Naval Medical Center San Diego Medical Home Port teams.

  • Work with all clinical teams as a resource on care management of all patients of the practice. Improving patient care, quality of life and compliance are the focus to include the following:

  • Pre-visit planning to ensure information and care completion prior to visit when possible.

  • After visit summary review with patients whenever appropriate.

  • Provide follow-up contact with patient to ensure compliance with recommendations.

  • Educating and involving patients to improve their health-patient engagement. This includes health fair and patient education classes which occur once a month.

  • Assisting patients to set goals, progress and identify barriers when patient not meeting treatment goals.

  • Serve as the resource across the continuum (consulting physicians, hospitals, ER and other frequently used healthcare resources) and the patient. Manage many aspects of patient's care: referrals, hospitalizations, ER visits, ancillary testing and other as directed to achieve optimum outcomes in safe and cost effective manner.

  • Develop a list of medical supply and community resources available to patients. 

Registered Nurse (RN) - Lead Health Educator Job Qualifications:

  • RN Licensure: current unrestricted in any U.S. jurisdiction
  • BLS
  • Master's Degree in Health Promotion, Education, Community/Public Health or Wellness, or a Health Science related field from an accredited university
  • At least 4 years of experience within the past six years of full-time experience as a health educator working with Health Promotions and Wellness programs.
