The Registered Nurse (RN) - Disease Manager job is within the military medical system and is a civilian position. Civilian and military personnel work together in the extensive military medical system. The hospitals and medical clinics provide services to the active duty military service members, military retirees, and their families and provide a full range of medical services.

Registered Nurse (RN) - Disease Manager  Job Overview:

  • Utilizing major components of population health and disease management
  • Ensure health care interventions for individuals and populations of patients with prevalent and/or chronic diseases in order to achieve disease management compliance
  • Improved overall quality of life and reduced healthcare costs.
  • Performs clinical nursing care in the execution of disease management services.

Registered Nurse (RN) - Disease Manager Job Qualifications:

  • BLS
  • ACLS
  • Associates Degree of Nursing (ADN)
  • Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
  • RN Licensure: Current, full, active, and unrestricted. Any U.S. state honored. 
  • Certification/Education preferred in disease-management related area such as:
  • Chronic Care Professional (CCP) by Health Sciences Institute
  • Certified Health Coach (CHC) by National Society of Health Coaches
  • Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators
  • Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board

OR

  • Certification eligible with 3 years for ADN or 2 years for BSN of full-time disease management experience
