Registered Nurse (RN) Case Manager Job Overview:

  • Strongly preferred working knowledge of the military health care system including TRICARE health
  • Coordinates patient care through a continuum and facilitates the achievement of optimal outcomes in relation to clinical care, quality and cost
  • Ensures compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the medical treatment facility. 
  • Ability to effectively communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
  • Excellent oral communication skills for patient/family education and telephone triage of patients.
  • Telephone and in-person assessment/evaluation includes the determination of optimal time and location for patient management (ER, clinic, homecare) and follow-up care as required.
  • Prescribes and communicates treatment plans and patient teaching in accordance with established protocols.
  • Knowledge of professional care theories, principles, practices, and procedures to perform nursing assignments of moderate difficulty and experience in assessing pediatric, adolescent, adults, obstetrics, and geriatric patients, as appropriate.
  • Utilizes the nursing process as a basis for professional Knowledge of a wide range of medical disorders/conditions and disease processes across the lifespan, asappropriate.
  • Knowledge of preventive health schedules, procedures, processes and education
  • Knowledge of disease management and population health
  • Knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required.
  • Knowledge of the administrative requirements for proper documentation of patient conditions including disease progress, acknowledgement of teaching, and follow up
  • Knowledge and proficiency with computer operations and platforms

Registered Nurse (RN) Case Manager Job Qualifications:

  • Certification from one:
    • Commission for Case Manager Certification Certified Case Manager (CCM) Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS) 
    • Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN) 
    • American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S). 
    • National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC) 
    • Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC) 
    • American Nurses Credentialing Center Nurse Case Manager (RN-NCM) 
    • National Academy of Certified Care Managers: Care Manager Certified (CMC) 
  • RN Licensure: Current, full, active, and unrestricted license.
  • Demonstrate clinical competency within the past two years in Case Management
  • Proficient in Microsoft Office software
  • ALTHA, CHCS experience highly preferred
  • At least 3 years of experience in a primary care clinic
  • RN License: Current unrestricted. Any U.S. state honored.
  • BSN preferred, minimum Associate of Nursing degree from a NLNAC or CCEN accredited program
