Registered Nurse (RN) - Lead Health Educator Job Overview:.

  • Perform management and coordination of TPS program.
  •  Liaise with the Defense Health Agency (DHA) with regards to the outcomes and measures of the program.
  • Identify TPS candidates using the Military Health System Population Health Portal (MHSPHP) component of CarePoint using the Rising Risk Filter and adjusting the Total Cost Band variable to capture cohort of patient.
  • Introduce the program, along with Site lead, to a new MHP for program expansion provide introduction and explanation of the TPS through a Medical Home Port Population Health Primary Care Manager Brown Bag Session PowerPoint. If possible, include IBHC and available clinical pharmacist.
  • Coordinate the logistics of the Kick-off IDT Meeting.
  • Inform PCMs and staff of the ability to provide reactive referrals.
  • Facilitates the meeting with support from the Health Educator(s), and additional providers such as IBHCs, RNs, and clinical pharmacists invited by individual sites.
  • Use site-specific materials as appropriate (e.g., recruiting scripts) to recruit patients to participate in the TPS.
  • Administrative enrollment of patients during the recruitment engagement, such as completing the HRQOL and Lifestyle Questionnaire and complete an enrollment TSWF T-CON.
  • Administer consent to participate, Health-related Quality of Life (HRQOL), Health Lifestyle Questions, HIPPA disclosure and graduation survey
  • Schedule initial patient visit with Health Educator.
  • Provide Health Educator with a Welcome Packet to include RRS/Care Plan, initial paperwork, and HPW resources specific to COI.
  • Collect data to generate reports on cost savings, improved in health outcomes, and health system utilization for NMCP, DHA, BUMED and other inquiring entities. 

 

Registered Nurse (RN) - Lead Health Educator Job Qualifications:

  • Master's Degree in Nursing, Health Promotion, Education, Community/Public Health or Wellness, or a Health Science related field from an accredited university. 
  • 5 years of full-time experience as a health educator working with Health Promotions and Wellness campaigns aligned with the U.S. Surgeon General's National Prevention Strategy.

  • RN Licensure: active, current, unrestricted. Any U.S. state
  • Graduate of a state accredited professional nursing program OR a program accredited by ACEN or the CCNE
  • 1 year experience as an RN working in an outpatient acute care (hospital based) or ambulatory care setting within the last two years
  • AHA Basic Life Support (BLS) or ARC Basic Life Support (BLS) for Healthcare Providers or AHA or ARC Healthcare Provider Course
